چکیده

A mong patients undergoing diagnostic cardiac catheterization, transradial access leads to improved quality of life after the procedure, is strongly preferred by patients, and reduces hospital costs.1 Transradial coronary angiography and angioplasty is safe, feasible, and effective, with results similar to those of the transfemoral approach.2 Although complications are infrequent, they do occur (see Complications sidebar). It is imperative that physicians using the radial approach are cognizant of the incidence, predisposing factors, and available management strategies for such complications.

Identifying patients at risk of vascular access complications

Routine coronary angiography and revascularization in patients with non-ST-segment elevation acute coronary syndromes (NSTEACS) have been shown to be of benefit.1,2 However, these patients are at an increased risk of bleeding due to antiplatelet drugs, anticoagulants and invasive procedures. Major bleeding in patients with NSTEACS has been associated with increased risk of mortality.3-5 Vascula...

Radial Versus Femoral Access for Coronary Angiography/Intervention in Women With Acute Coronary Syndromes: Insights From the RIVAL Trial (Radial Vs femorAL access for coronary intervention).

OBJECTIVES The purpose of this study was to determine the efficacy and safety of radial versus femoral access in women undergoing coronary angiography/intervention. BACKGROUND The risk of bleeding and vascular access site complications are higher in women than in men. METHODS In a pre-specified RIVAL (RadIal Vs femorAL access for coronary intervention) subgroup analysis, we compared outcome...

Trans-radial versus trans-femoral access in patients with end-stage liver disease undergoing cardiac catheterization.

Cardiac catheterization has been increasingly utilized to evaluate coronary artery disease in patients with end stage liver disease (ESLD). It is known in other populations that radial access reduces access site complications;however, there is a paucity of data in ESLD patients. We investigated vascular and bleeding complications rates between trans-femoral and trans-radial cardiac catheterizat...

Managing Complications of Transradial Catheterization

D uring the 25 years since the first report of successful coronary catheterization via the radial artery,1 transradial access for catheterization and intervention has evolved from a trendy idea with sporadic adoption to a routine practice and standard of care. Following on the heels of small series and meta-analyses suggesting a reduction in bleeding and access site complications, larger studie...

Radial Artery Pseudoaneurysm: A Rare Complication after Transradial Coronary Angiography

The transradial approach has recently been recommended as the first choice to reduce access-site vascular complications during percutaneous coronary diagnostic and interventional procedures [1]. It may be preferred in anticoagulated patients to avoid those vascular complications associated with the femoral approach, which are compounded by systemic anticoagulation [2]. The radial artery pseudoa...
